Payment is due in full at the time dental treatment is provided.

1. We welcome cash, check, major credit card (Visa, MC, Discover), or Care Credit at the time dental treatment is provided.

2. As a courtesy to our patients with dental benefits, we will submit your claim to your insurance company.  Any portion not expected to be covered by these benefits is the responsibility of the patient and due at the time dental treatment is performed.  This amount will include deductibles and co-payments.  Please understand that this is only an estimate--not a guarantee of payment, and is based on the information available to us from your insurance company.  If, after 90 days, your insurance company has not paid its portion, the patient will be responsible for paying the balance in full.

3. The person who accompanies a minor/child to the appointment will be considered the responsible party and, therefore, is responsible for any payment due at the time service is rendered.  For unaccompanied minors, non-emergency treatment will be denied unless charges have been pre-authorized before the appointment date or previous arrangements have been made with the Office Manager.  Marital status is not a consideration under any circumstance.  Decreed custody or lack thereof, does not alter financial responsibility.

4. If dentures, partials, crowns and bridges are to be fabricated by a dental laboratory, a 50% deposit will be required at the time of the first impression. The remaining balance is due at the time the prosthesis is cemented or inserted.

Insurance Assistance
Our goal is to maximize your insurance benefits and make any remaining balance easily affordable. Please remember, however, professional care is provided to you, our patient, not to an insurance company. Financial Assistance
Our fees are based on the quality of the materials we use and our experience in performing your needed treatment.

Our goal is not to let expense prevent you from benefiting from the quality of care you desire and need. To facilitate this goal, we have developed the following financial options:

Option 1: Bookkeepers Discount
This option allows you to pay the total cost (when fees are in excess of $200) of the procedure up to one week prior to the start of the work and receive a 5% discount on the price. This also eliminates the worry of your insurance covering its portion within the time allowed. This option only allows payment by cash or check.

Option 2:  Outside Financing
We are pleased to offer outside financing through Care Credit. Those who qualify will use the Care Credit card as a form of payment at the time of service, and then payments are arranged through that company. This program is similar to a credit card and offers low monthly payments and flexibility to those who qualify.  This nationwide plan can also be used for vision, veterinary, cosmetic surgery, etc. at participating providers.

With Care Credit, you can get same day approval, so there is no delay in your treatment.  Please ask the front desk staff for further details prior to your appointment or visit Care Credit on-line at www.carecredit.com.

Option 3: Cash Discount
We offer a 5% discount when you pay cash in full at the time of service.

Option 4: Senior Citizen’s Courtesy
We offer a 10% discount to those patients age 65 and better. For those patients with insurance, your discount is taken off your estimated personal portion at the time of service.

Option 5: Major Credit Card
Major credit cards such as Discover, Visa and Mastercard are accepted in our office.
